Information clause (contact)

Below we provide the information required by Art. 13 GDPR regarding the processing of personal data in connection with contact with Instytut Medycyny Innowacyjnej sp. z o.o. (e.g. by email or contact form).

3. Scope of data

Depending on how you contact us, we may process:

  • first and last name,
  • email address,
  • phone number (if provided),
  • message content and any data you voluntarily include in it.

4. Purpose and legal basis

  • Handling enquiries and communication – responding to enquiries and correspondence (Art. 6(1)(f) GDPR – legitimate interest of the controller).
  • Consent – where we request it in a given process (e.g. checkbox in a form) (Art. 6(1)(a) GDPR).
  • Establishment, exercise or defence of legal claims – where necessary (Art. 6(1)(f) GDPR).

5. Recipients of data

Data may be disclosed to entities providing us with technical support (e.g. hosting, IT, email) – as processors under data processing agreements.

6. Retention period

We store data for as long as necessary to handle the enquiry, and thereafter for a period justified by archiving and potential defence of legal claims.

7. Your rights

You have the right to:

  • access your data and obtain a copy,
  • rectify data,
  • erase data (in cases provided for in the GDPR),
  • restrict processing,
  • object to processing based on Art. 6(1)(f) GDPR,
  • withdraw consent at any time (where processing is based on consent).

8. Complaint to a supervisory authority

You have the right to lodge a complaint with the President of the Personal Data Protection Office (UODO – Urząd Ochrony Danych Osobowych), if you believe that data processing violates GDPR provisions.

9. Voluntary nature of providing data

Providing data is voluntary, but necessary to handle the enquiry and provide a response.
